i tried the Keter difficulty myself to see if it worked because i see videos from later updates (i do not know in which update it stopped working well, must be a longer time ago). In the early updates it was functional but now it is not real working.
So:
-Keter difficulty selected
-skipped the intro
-press F5 and it says "Game saved"
-if you die is the only thing you can do "Quit"
-in the Menu you can click "Load Game" and ... LOAD THE FILE

and should the Blinking and Sprint Meter not supposed to be off ?

That's pretty much new. Probably the game failed to delete the save when you died.

And you can turn off the HUD in the options menu.
