It'd have a 47% chance to kill you, but have a total 53% to alter the player character's abilities, each alteration having its own rarity of being applied: more time between automatic blinking, faster run speed, or less fatigue, for positive examples; less time between automatic blinking, slower run speed, or getting fatigued more quickly, being negative examples. Or it could do something asinine and not effect gameplay at all, like adding an extra ear or something stupid like that.

Also, to preventing it from being OP, it could only be used once before something happens, causing it to break down or something.

Yes, I know it could be exploited via saving like 914, but if the developers were concerned with exploiting SCP–914, something would have been done.

Sounds like a good idea but I don't know. Sounds good enough for me.
