I don't think there are any other blast doors which are keypad locked.

There's one in the room where you find 682's document and one in the 049 tunnels to i believe.

Pretty sure he has a spawn timer, maybe by coincidence it ran out while you were in there.

This isn't very subtle but what if the room turned upside down after awhile, like if you were walking on the ceiling?, it would only happen once.

I've noticed the falling through the floor thing happens a LOT in those straight hallways in the LCZ, usually around in the center of the room. I've seen a video with the 939 room and there is elevator shafts that look like you can fall down them, maybe the elevators bugging out as if you're falling...

I went down 939's elevator like omniary did and died with the same noise, but i saw 106's glowing eyes for a second so i thought it was an elevator trap or something, wasn't until a video i realized it was leading to 939.
