Why doesn't Barotrauma use steam servers.
I don't understand what you mean by that.
And creating a server is pretty straightforward.
Games like factorio, ark survival, space engineers, stellaris, terraria, and games with multiplayers like these all give you the ability to create a small co-op server/server where you can steam invite your friends without the need to port forward on your router. It means that those 4 or so friends who want to play campaign can do it with ease. And the ability to create servers easier would mean small groups who just want to play can avoid trolls.
Many games use these dedicated servers:

Getting rid of the port-forwarding thing is on the devs todo list, it will get resolved in time. For now this is how it works, Early Access and such.

Also, you are confusing what dedicated servers are. Barotrauma has dedicated server setup, it's just means a bit different thing, from the one you're thinking about.

Those dedicated servers have nothing to do with that. What you are talking about is P2P connection, which isn't implemented yet.
But I get what you mean, and I'm sure that it will be implemented eventually.
