Yeah that looks like a photoshop. One of the MTF's is indeed shooting the player it would seem... adding names above their heads plus the little "health bar" on your left would be pretty easy to shop into the picture.
But SCP-CB Multiplayer would be alot of fun....

I am nearly certain that it is a fake image. I saw it a long while ago.
