Unfortunately, Regalis is on leave for a while, but you might get an answer in the next couple of weeks.

Oh he wanted a blank sheet, if I knew that...

However as Omniary said Regalis will be off for a while and he will not have regular and consistent communication with these forums. But I am sure we can still help.

Do you have the raw PS file and details of how the notes are made? I need to make notes on SCP-008 and 005 but I need the right font and the template :3
