Exactly, when I get cautious I think I see "crap" everywhere, crap being something creepy and or awkward, for just a second. Does anyone else here get that?

I thought you were talking about the type of insane where the things you see can ACTUALLY hurt you. (it happened in real life, no jokes)

It'd be interesting to see hallucinations in parts of the game where reality is challenged, like in the pocket dimension or the forest.

Maybe there could be a dark corroded human figure (like the 106 victim) occasionally appearing briefly down dark corners and running across the stone path in the Pocket Dimension? It could work like 513-1 in a sense except when you look at it at all it'll disappear.

And maybe there could be more hallucinations appearing on screen? (like 106face)

Anyone remember hg.pt? You know, the weird ghost that appears sometimes in the void?
