Memory Access Violation (SCP-049 Idle)

I'm well aware that you guys pay more attention to MAVs which occur the second the game boots up, but I thought maybe somebody might have encountered this instance of MAV at some point. Anyway, I take the elevator to SCP-049's floor and go to his containment chamber so that I can lower the two levers and go back up, simple enough. The problem is that when the door opens and SCP-049 enters his idle ... state or whatever the MAV occurs. I can testify that's what happens because I walked right in front of him right after pulling the first lever and he tried to attack me, but the second I lost him the MAV instance occurred.

I've been playing on V1.1.5 until today that I've downloaded V1.2.3 and this never happened to me before. I also tried playing through the game with the same seed on a new save file and the MAV still occurs. If anybody knows of a way to fix this please let me know.

Other tested fixes that haven't worked:
  • Right click the .exe file, select 'Run as Administrator'.
  • Right click the game's folder, select 'Properties', untick 'Read Only', click Apply, then try running it.
  • Right click on the .exe, select 'Properties', go to the 'Compatibility' tab, set compatibility to XP Service Pack 3.
  • Kill "taskhost.exe" process in your task manager.
  • Try running the excutable directly instead of using shortcuts.

I know this is considered a Double-post but I was checking out the game's files and I don't know if there are supposed to be SCP-049 files under <game path>\GFX\npcs, because if there are I see none. If anybody has those missing files do you mind to post them here? It'd be of great help.
