1b. You make your own image in the resolution 350x19.
2. You use this site to adjust the image to your liking in order for it to become a proper userbar. I recommend using the "Overlay glass shine" option.
3. Once you're done making a complete userbar, save it to your computer and upload it onto another site (ImageShack, Imgur etc.) and then put it as an image in your signature. If you want multiple userbars, they should have this format:

Keep in mind that your signature space isn't infinite, so be conservative of what you have.
