Yes, there 682 does indeed make a legitimate appearance when he decimates the helicopter, depends on where you are to see it best. If you're on the walkway, it's easier to see its claw hitting the helicopter, but the metallic grids over the walkway somewhat obscure the view. In the control room, you can see everything clearly, although the wall separating the Gate and the hillside makes it a bit hard to see.

With the fog though, it's quite hard to see it, it appears to just look dark and untextured when afar.
He doesn't have a model yet, it's just an arm.

He doesn't use the pathfinding system anymore. He just floats through walls towards you, like in the older updates.

I am sure its just this guy was just exiting the old mans realm by normal means. However at 25:50 It would Seem 096 Still attacked him even in the realm, Even if he pissed it off earlyer. I am sure like I said in the start of this post its just him spawning in the terrible place but, if it is true that 096 will still go after you if you enter other worlds that adds a whole new element of horror to it.

His little Squueak entertained me for hours seriously I kept replaying it over and over just to hear it. It was just too flipping funny.

Found a couple of model files (thanks to Destructoid) in the game files. The ones that intrigued me most were the 650 chamber, which I always though to be 173's (it was in versions way back, like 0.2), and MTF's with Gas Masks. They actually look quite terrifying.
