A lot of people know about SCP-049 all too well, and they also know about SCP-049-2. With 049 being the Plague Doctor himself and 049-2 being the human that 049 has performed surgery on. The SCP Foundation are secretive with a lot of things, and they may not want people to know about 049-1 and what it truly is (if 049-1 exists). However, I have a couple of ideas on what 049-1 might be:

The substance on 049's hands (gloves?) - Perhaps an obvious choice is the deadly touch itself. After all, the foundation mention his touch causing so 049-1 could easily be something causing an unknown affliction that kills a person within seconds. Since it's probably not cardiac arrest, the lack of knowledge on this affliction makes it anomalous, therefore making it an easy choice.

The human 049 is currently pursuing - It is a possibility that the 049-1 mark is given to a human who has crossed the Doctor's path, much like a human who views 096's face is referred to as 096-1 as long as 096 is after them. In this sense, you can see why this is one of my choices.

The material 049's surgery bag is made of - A less obvious choice. But while the bag may be made of a mysterious human-known element that the Foundation want to keep a secret for whatever reason, it is probably more likely made of an out-of-this-world material that 049 is able to create. Thus, this seems like a reasonable choice.

Any other suggestions as to SCP-049-1 are welcome!

I don't want to make it sound like you made a useless thread because you over-thought things, but you made a useless thread because you over-thought things. It's pretty obvious that SCP-049-1 is the plague doctor since he is the main entity. SCP 049-2 was only named that since it is a product of the main entity, the plague doctor. If there was some other SCP-049-1, it would have referred to it in the article, especially since all of the items had been mentioned in the article.

Pretty much this. The article was made back before there was a standardize system for naming anomalies which sprung off the main anomaly.
