So just throwing out ideas here, I think the idea of having the Shy Guy wandering around is kinda awesome, after the initial encounter. Just in random shady spots, you could see him sort of squatting, then you have a split second to tap the spacebar key before he turns around to sorta look over his shoulder at you.

A containment room would be awesome, as other people have suggested. As to how he got out - maybe have some rot stains in his chamber, like the Old Man accidentally, or with intent, melted his metal cube?
For the chamber, there is no need to have it be rotted away. The way it should be displayed is him just being out of his cell, and the cell itself broken as if he had escaped. I think it would be better if we had no explanation of how he escaped. After all, SPCs are popular because of their mysterious nature.

Well, since 106 and 079 are the ones responsible for the breach it should be either one. Although rotting his containment chamber would possibly put 106 at risk of viewing 096's face.
So it's more likely that 079 released some of the locks of its chamber and it just ended up wandering out.

096 has no drive to attack anyone unless he has his face viewed. So it makes sense that he's just casually taking a stroll through the facility while having a snack here and there

Yeah, its true that they are the cause of the breach. I'm just saying that we should leave it to the player to guess how he escaped. There should be a small "hint", per say, of how he escaped, but not just tell it directly.

Personally, I find it more scary if he burst out of his cell because he was triggered rather than let out. That gives the impression that there might be a picture of his face out there. Even though there isn't, that doesn't stop your brain from assuming the worst.

Although I like the idea of hearing the actual breach of 096 somewhere closeby, it wouldn't make much sense.
There is no video surveillance etc. of any kind near is chamber.

UNLESS of course, the picture in that one 096 incident got revealed to staff and such. (It was never stated the picture was destroyed, so it's possible that it got contained as well)
That could actually work in many ways. But it would take lots of scripting.

No, what I mean is, that you see the chamber already breached, not it being breached. This way, it would take less scripting, and would make the player's mind turn itself out coming up with the possible reasons of how it escaped.

This way, you could think that it was 079 who released him, or that there is a photo going around, and if you think about it, it would make sense that the foundation kept one photo well locked in case they needed one, since taking a photo of it would be quite difficult.

I suppose it would make sense. But you'd also have to take more precautions when introducing 096 in the future.
Right now it's "READ IT UP ON THE WEB OR DIE!" and that just can't be right. Every single SCP has its own paper describing the Item/Creature.
So 096 is a newbie-trap. Not that 173 isn't one... quite the opposite. I've seen SO MANY people fail.. but oh well. Diffrent topic for 173.

Players need a warning for 096, and of course it needs its own containment chamber. Also, the idea of encountering the place after he breached would work better anyway.
He already has animations for leaning against a wall, shielding his face before looking left and right. (And of course, suddenly turning around!)

I'm dying to see those ingame.

Well, you have the documents ingame that give you all the information you need. Don't tell me those aren't enough, or that you need better introduction, because if the player is too lazy to read them, then they deserve to get killed by 096 until they learn it the hard way. The documents are the tutorials of the game, except they don't interrupt gameplay, and make sense to have them there.

Other thoughts: Maybe as a way to mitigate Shy Guy would be a bag or hood, as described in it's file? Think about it - trying to peek out from behind something, or glimpse a corner, to see if it's turned around, then slowly creeping over to it, closing your eyes each time it gets curious and turns to look at you, until you're close enough to tie the hood over it's face.
