That means that at least one of these files are missing from the game's directory:
  • zlibwapi.dll
  • FastText.dll
  • FastExt.dll
Before you say that you already installed it correctly, make sure you actually did. If you actually did install the mod correctly, then it should work.

Well... I'm pretty sure that the problem here is that you haven't installed it correctly. It has worked for everyone else, and as juan said, if you had installed it correctly then there wouldn't have been an issue. That's kinda how it works. Just try again, but this time be more careful to follow exactly what the instructions say. It's pretty straightforward.

-------- BOX OF HORRORS: INSTALLATION GUIDE --------

1. Download the latest version of the mod

2. Open the downloaded file in an archive extractor
(WinRAR, WinZip, Windows Explorer, etc.)

3. Open your SCP - Containment Breach folder

4. Drag all of the files from the downloaded folder
to your SCP - CB directory

5. Accept all requests to replace files and merge
folders

6. Run "SCPCB with Box of Horrors vX.Y.Z.exe"
(X.Y.Z = version number)

------------------ FIXING ERRORS -------------------

- General errors:
	- Memory Access Violation!

	This is a common Blitz3D error. Try running
	the executable as Administrator or setting a
	compatibility mode. Also, make sure there
	are no missing files, and the game can read
	and write files in its directory.
	This can also happen if you try loading a
	corrupt or incompatible save, or find an
	actual bug with a specific event.

- At Startup:
	- User lib not found

	This means that at least one of the extensions
	this mod uses is missing or corrupted:
		- zlibwapi.dll
		- FastText.dll
		- FastExt.dll
	This usually happens when the mod is installed
	incorrectly.

You have to go back and install the mod correctly, as described in the first page.
