So it appears that the second image is a 1123 hallucination, the note looks like something out of the holocaust (it appears to be a baby with pencil scratchings on it to make it look like a mutilated baby) and the 'Heavy Containment ClassD cell' appears to just be some sort of cell they would throw victims in.

The first image is most likely something from the PD, it probably adds onto that one big shadow that appears in the PD behind the player at random moments. Like a corpse of some kind.
The preparation for a dive is always a tense time

I'm pretty sure he's alluding to the fact that there's more factor of chance than exploration required to see the new room.
It's escapable, yes, but some people who've been in the dimension numerous times have never even seen the planes. If you're in there, you'll bleed out before you have time to find rooms.
